Please sign as a parent/guardian on behalf of any U18 entry. Max. entries per event are indicated on the entry "tick boxes". No entries will be accepted without this form & the full fee. Please fill in full details for each player, which is important for contact details. CLOSING DATE FOR ENTRIES is 31ST JULY 2026 (or when events are full, a waiting list will be created)
